General Instructions: 

• Complete 2024 EMT Student Application
• Fill out this form (if applicable*) - Verification of Membership in a NYS EMS Agency DOH-3312
• Submit ALL completed forms with tuition and lab fee to:

• Payment Options:
  • Cash
  • Check/Money Order payable to Town of Colonie with student name and EMT Course in memo
  • Online Debit or Credit
    • *Online payments service fees are as follows: all debit and credit cards: 2.75%.

*Not sure if your EMS Agency qualifies for reimbursement?
