General Information and Instructions:
- Tuition: EMT Original Course cost is $950 tuition, plus a $150
lab fee if paid with check/money order (see below for debit and credit card pricing)
- If you are a member of an approved EMS/FD agency AND submit a verification of membership form, the tuition is waived.
- If tuition is waived, you are still responsible for the $150 lab fee.
- If you are a member of an approved EMS/FD agency AND submit a verification of membership form, the tuition is waived.
- Complete Course Application
- Fill out this form (if applicable*) - Verification of Membership in a NYS EMS Agency DOH-3312
- Submit ALL completed forms with tuition and lab fee to:

- Payment Options:
- Check/Money Order ($950 tuition, $150 lab fee) payable to Town of Colonie with student name and EMT Course in memo
- Online Debit or Credit :
- ALL debit and credit card payments have a 2.75% surcharge
added upon checkout*.
- Total charges for debit/credit payments are as follows:
- $150 = $154.13 charge (lab fee if tuition is waived) $1100 = $1130.25 charge ($950 tuition plus $150 lab fee) *Prices in bold represent total charges for tuition/lab fee after the surcharge is applied upon checkout - DO NOT enter the prices in bold when paying online!*
